This guide is designed to help students and their families, local education agencies, teachers, communities, and State agencies facilitate the movement from school to post-school activities.  It also provides technical assistance in the appropriate implementation of the transition requirements of IDEA 2004.  Three training modules are designed to accompany the guide: (a) Service Provider Training; (b) Administrator Training; and (c) Family Workshop.  Links to the modules are shown first; the link to the guide is beneath them. This self-paced web module and referenced approach will assist the learner in understanding the Individuals with Disabilities Education Act (IDEA), as amended in 2004, and the components of a performance goal and how this format applies to the mandates of the IDEA.  A step-by-step approach directs the learner through goals and objectives, their relationship to district general education standards, and practical methods of measuring objectives to assure progress.
